    • Aus der Sicht der Neurowissenschaften sind Erkrankungen des autonomen Nervensystems erst in jiingster Zeit ins klinische Blickfeld geriickt. Trotz einer langen Forschungstradition am autonomen Nervensystem die mit Namen wie Cannon, Hess und MacLean verkniipft ist bleiben die Kenntnisse iiber Struk tur und Funktion bei wei tern hinter jenen zuriick, die gerade in den letzten lahren iiber sensomotorische Systeme gewonnen wurden. Die autonome Diabetesneuropathie stellt auBerdem nur einen Sektor aus einer Vielzahl bislang wenig beriicksichtigter metabolischer, toxischer und entziindlicher Storungen am vegeta tiven Nervensystem dar. Diese Form der Neuropathie ist zugleich mit besonders vieWiltigen klinischen Symptomen verbunden. Auch aus der Sicht der Diabetologie wurde die autonome Neu ropathie in Klinik und Forschung bislang nicht ausreichend beriicksichtigt. Dies diirfte einerseits dadurch bedingt sein, daB die klinische Bedeutung der autonomen Neuropathie als Kompli kation des Diabetes mellitus unterschatzt wurde und andererseits methodische Schwierigkeiten bestehen, neuropathische Verande rungen bestimmter Organsysteme friihzeitig zu erfassen. Die Untersuchungsmethoden zum Nachweis vegetativer Storungen stammen dabei nur teilweise aus Neurologie und innerer Medizin, groBenteils aber aus anderen Fachgebieten, wie z. B. der psychophysiologischen Forschung. Es handelt sich somit urn ein Grenzgebiet, das die Integration internistischer, neurologischer und endokrin-metabolischer Aspekte erfordert. Zudem stellt der Diabetes mellitus mit seinen Organkomplikationen wie hier am Beispiel der autonomen Neuropathie gezeigt, auch ein Para digma fUr psychophysische Wechselwirkungen und zugeordnete psychologische Probleme dar, auf die hier ebenfalls erstmals naher eingegangen wird."
