Diagnosis and therapy of pancreatic diseases is still a challenge for the gastroenterologist. For instance, sufficient screening of pancreatic cancer is to date not available, and the identification of risk factors and at-risk patients is an important step towards the diagnosis of the disease: For example, it is a well-known fact, that chronic inflammation - in this case chronic pancreatitis - plays an important role in gastroenterological carcinogenesis and may be involved in promoting cancer growth, tissue invasion and metastasis. On the other hand, hereditary pancreatitis as well as other gene mutations plays a critical role in the etiology of chronic pancreatitis, whereas alcohol, as well as other factors, has a minor influence on the development of this condition. The publication at hand examines various aspects of pancreatic diseases, featuring contributions on the etiology, diagnosis, classification and treatment of acute and chronic pancreatitis, as well as on the epidemiology, risk factors and thrombotic complications of pancreatic cancer. It will be a valuable source of information for the clinical gastroenterologist faced with pancreatic disorders.
Acute gastroenterological conditions are associated with significant morbidity and mortality, necessitating close collaboration between gastroenterology and surgery for optimal therapy. However, a systematic approach to diagnostic procedures and treatment has been lacking. This publication aims to bridge that gap by offering recommendations for a rational multidisciplinary approach. Acute conditions often involve multifactorial etiologies and polymorbidity, which can negatively impact diagnostic accuracy and treatment efficacy. In this context, endoscopic examinations play a crucial role, allowing for simultaneous therapeutic interventions. Yet, endoscopy is invasive, and patients' complex health conditions can limit its feasibility, particularly regarding cardiopulmonary status. Consequently, non-invasive diagnostic methods such as ultrasound, CT, or MRI are essential alternatives. Innovations like CT-enteroclysis and CT-colonography offer precise, immediate results that can guide optimal treatment strategies. This publication is tailored for gastroenterologists, endoscopists, surgeons, intensivists, and other physicians managing acute cases, providing them with the necessary tools to determine the best therapeutic options for their patients.
Nová monografie interny pro studenty a lékaře v „interním kmeni“ je zaměřena především prakticky. Autoři v ní poskytují stručnou a srozumitelnou formou aktuální, prakticky orientované informace o klinickém obrazu, diagnostice a léčbě interních onemocnění. Opakování patofyziologických aspektů chorob je omezeno na minimum nezbytné pro pochopení klinických projevů a léčby. Monografie pokrývá v základním rozsahu celé spektrum chorob všech interních oborů. Zvláštní pozornost je však věnována chorobám, se kterými se lékaři v obecné interní praxi setkávají nejčastěji, a problematice akutních stavů – zde monografie poskytuje dostatek zcela konkrétních a prakticky orientovaných informací, aby mohly být vodítkem pro jejich diagnostiku i léčbu. Dvě závěrečné kapitoly jsou věnovány praktickým problémům v interně, které přesahují rámec jednotlivých interních oborů, a vybrané problematice z jiných lékařských oborů. Autorský kolektiv zahrnuje pracovníky většiny fakultních pracovišť České republiky. Mimořádným úsilím autorů i redakce vznikla kniha v období kratším než jeden rok. Důraz na srozumitelnost a praktickou použitelnost ovlivňoval práci autorů od samého začátku a texty vznikaly s pomocí připomínek oponentů, kteří byli nejen z řad mladých lékařů, ale i studentů posledních ročníků medicíny.
