Many extrapyramidal motor diseases stem from advanced neurodegenerative processes by the time of clinical diagnosis. Animal experiments suggest that neuroprotective substances may soon be available, emphasizing the need for early therapeutic intervention. This premise inspired a meeting at Chiemsee, aimed at developing instrumental methods for earlier diagnosis, leveraging expert experience, and improving therapy resolution compared to current practices. As early as 1886, Charcot attempted to differentiate tremors in multiple sclerosis and Parkinson's patients using a drum created by physiologist Marey. Over time, various devices emerged to identify movement disorders, primarily for diagnostic purposes, but many proved less effective than skilled observers. However, recent advancements in measurement techniques and evaluation methods over the last two decades have significantly enhanced the quantification of motor disturbances. The symposium at Chiemsee and this book aim to provide an overview of the commonly used procedures for extrapyramidal movement disorders, highlighting their potential and limitations.
